YEARLY MEDICAL FORM

Every year we require our clients to fill out a medical form on their pup/dogs health.  This helps us in making better choices for breeding our animals as healthy as possible.  While things happen that are out of our control, (accidents, cancer, birth defects) there are a lot of things we can control.  But only if we know there is a problem.  There are also other minor things (ear aches, allergies, UTI) that we can give advice to help stop or get under control.  Please CLICK on the BUTTON below to access our Yearly Medical Form.
